Judge ZEMAITIS, 6TH FLOOR, COURTROOM 6-C

An initial and/or supplemental child protective petition has been filed in the above matter. A hearing on the petition, including petition to terminate your parental rights and a permanency planning hearing, will be conducted by the Court on the date and time stated above in the 17th Judicial Circuit Court, Family Division, Kent County Courthouse, 180 Ottawa NW, Grand Rapids, Michigan. The permanency planning hearing will result in the child(ren) being returned home, continued in foster care, or the court may order proceedings to terminate parental rights. IT IS THEREFORE ORDERED that you personally appear before the court at the time and place stated above and exercise your right to participate in the proceedings.

A preponderance of the evidence establishes that the natural father of the child(ren) is unknown. Any unknown father must establish legal fatherhood of the child(ren) within 14 days from this publication by either signing an Affidavit of Parentage with the child(ren)’s mother, or scheduling DNA testing and providing a tissue sample, or initiating a paternity action.

Failure to establish legal paternity, as stated in the above paragraph, by the time of the above stated hearing, waives all rights you have to further notice of any proceedings involving the minor child(ren), including notice of termination of parental rights and the right to an attorney. The written Advice of Rights to Putative Father is available at the Case Management Department of the Kent County Circuit Court.

THIS HEARING MAY RESULT IN THE TERMINATION OF YOUR PARENTAL RIGHTS or in a temporary loss of your right to the child(ren).
